Position Overview
The Service Coordinator’s primary purpose is to help connect residents with resources they need to maintain and improve their lives. Resources and assistance is provided to all residents in the following areas:
- Placing / translating calls
- Physician appointments - booking, billing, and general information
- Navigating health Insurance /benefit plans
- The Social Security Administration
- Living will / legal issues
- Obtaining transportation
- Franklin County Board of Developmental Disabilities
- Franklin County Job and Family Services
- Ohio Department on Aging
- Home Health Agencies – housekeeping, in home medical care, etc.
- Group outings to Wal-Mart, Kroger, and other local stores
- Educational workshops on varying beneficial topics
